in later years.

Some patients who underwent stomach surgery to reduce the present, after five years or more, a considerable new weight gain. Moreover, other disturbances are also being observed in the same period after the operation, including alcoholism, anorexia, bulimia, bruxism, excessive increase of dental caries and brittle.

The information is being collected and interpreted by a group of multidisciplinary study of the Clinic Hospital(CH), Faculty of Medicine, USP. The objective of the study, still in progress, is to demonstrate that only the surgery, which is made in the hospital for nine years, not just for the success of treatment used for the drastic reduction of the weight.

According to psychologists, a group of patients operated by the technical Fobi-Capella between five and nine years ago, 13% returned to a state of morbid obesity with a body mass index (BMI) over 40. The BMI is obtained dividing the weight of the person by height squared. In total, 64.15% again be obese with a BMI greater than 30.

After surgery, it is expected that the patient weight the desired quantity and then ten kilos weight again. But among the 53 patients studied, 58.5% gained more than ten kilos, 39.6% more than twenty kilograms and 13.2% fattened more than thirty pounds. Only 7.84% of the patients had ideal weight or emagreceram too (in cases of bulimia and anorexia).

“It is gross and results have not yet been made statistical studies. The data were not correlated with the organic matters and integrity of surgery. But it is nevertheless a warning to patients and doctors: the surgery should not be understood as a formula magic, “is said

According to the doctor and coordinator of the group, the study aims to show that the operation is not the end of treatment. “These patients may replace a compulsion for another. The secret to success is monitoring,” he explains.

For Marlene, the case of alcoholism, observed in 18% of those 53 patients studied, is one of the examples of the exchange of compulsion. People are beginning to enjoy the social benefits of weight loss - unlike the previous condition, in which they do not fell outside the home - passing thus to drink excessively.
Some patients who underwent stomach surgery to reduce the present, after five years or more, a considerable new weight gain. Moreover, other disturbances are also being observed in the same period after the operation, including alcoholism, anorexia, bulimia, bruxism, excessive increase of dental caries and brittle.

An obese person can be cured and that is normal weight or close to normal, but this does not mean that obesity has been cured. The only way of survive from obesity is to change for lifestyle, both in eating habits and physical activity. When an obese reaches the level of health threatening obesity there is a problem of very difficult solution, It normally has already demonstrated the failure of controlled medical treatments.

In morbid obesity,

the real possibilities of an effective treatment (a significant loss of weight) and sustainable in the medium or long term (maintenance of weight lost at 5 years or more) are less than 3%.

97% of obese morbid continue morbid despite being obese medical dietary treatment- (weight loss surgerydiet, medications, physical exercise psychological reinforcement, etc.).

After medical treatment dietary well-done it is possible that morbid obesity get a significant weight loss

but the rule is that it will eventually produce the recovery of weight and lost some kilos more (rebound effect).

For a morbid obesity a significant loss of weight is difficult but possible, what is truly extraordinary maintain in the medium or long term that weight loss. In the best case a few get a notable reduction of weight, weight recovered and almost certainly exceed, not being able to maintain proper eating habits and exercise (new lifestyle - healthy lifestyle).

It involves removing vertically in the left portion of the stomach and especially the bottom gastric here because there is a substance that stimulates the appetite. These patients are therefore a smaller stomach, around 150cc (the normal measures about 1200cc), but also reduces the appetite. It is a low-risk surgery and maintains continuity of the stomach with the rest of the intestine, unlike the gastric bypass.

This is a technique that is used for quite some time combined with a derivation bilio pancreatic, or a type of bypass. However some very obese patients and risk being made as a first stage of this type of bypass only the downsizing of the stomach in this form of manga, as a first step, thereby losing weight as to complete the surgery in a second time with less risk. However in some cases was that under long enough weight to the pure resection sleeve and managed to maintain proper control their weight low.

Thus has begun to propose this procedure as the only technique to lose weight, and if everything does not give the expected result is complemented by referral. The final results are under evaluation.

The intragastric balloon consists of installing a balloon in the stomach. It inflates with 500cc liquid stained with methylene blue. This procedure is done with general anesthesia and the introduction is by endoscopy. The operating mechanism is that the inflated balloon takes up space in the stomach and produces a feeling of satiety with little food because there is no more. The ball must withdraw in six months because after that time can be broken and can cause a blockage in the gut when moving beyond the stomach. The object of fill it with methylene blue is precisely because if it breaks the ball because the dye exit through the urine and will allow us to immediately withdraw the ball before advancing beyond the stomach.

This technique is useful for reducing from 10 to 15 kgs. It is not useful for morbid obesity. It is important that during this period the patient support with nutritionist and sometimes with psychologist to develop new eating habits and exercise, because if not to withdraw the ball inexorably recover their overweight.

This ” Gastric Band ” (The balloon) is composed of silicone, and occupies an area of 400 to 700 ml which will be completed by saline (not that dangerous substance to our body).

Gastric banding is to encircle the upper stomach with a kind of belt that divides the stomach in an upper small, just 15cc and leaves a small step to the rest of the stomach. This banda or ring is a device that allows more or less inflate the cuff so as to control the size of step. This fills the first upper stomach with what the patient is satisfied with little food.

The fact that it must ingest solid foods or liquids papillas pass easily through communication and not provoke feelings of satiety with little food.

The results are quite satisfactory if the patient has been well selected and meets the necessary instructions to their success. It requires the support of our team.

This we do laparoscopic surgery, is rather more simple and less risky that the bypass.

The data about dental disorders are unprecedented and show that half of the patients returned to the doctor with any complaint. About 80% was with the teeth brittle and had a 60% increase in the number of caries. These problems were observed both in patients from the Unified Health System (SUS) as of private health plans, which shows that the situation is somewhat related to socioeconomic class of patients

According to the dentists , the exact reasons for these problems are still being studied. “But this may be related to a problem in the absorption of nutrients already observed; refluxes with gastro-esophageal, with a drying mouth, the result of medication administered with or vomiting.”

The stomach-reduction surgery is one of the main themes of 32 of Gastric, an event with the participation of nearly a thousand medical specialists in surgery of the digestive tract. In the event, which began in the second (4th) and vai until Friday (8), the Convention , in Sao Paulo, there will be conferences, round tables and transmission of live surgeries.
